Forms and Surveys
Gurita also has a lighter-weight Survey tool, separate from guest forms, for questionnaires you send after a stay, satisfaction surveys, or one-off questions that aren't part of a guest's core profile.
Form definitions
Form Definitions are the question sets you build once and reuse for every survey you send.

Who can access this: Requires the Form Definitions update permission and the Forms module.
A definition has a name, logo, header colour, an optional customer-facing print template and an internal print template, and an ordered list of items. Each item has a type:

Any item can be marked Required, restricted to specific properties, and shown only if another item was answered a specific way.
Each item also carries a question and a longer question description per language. Media items (Image, Gallery, Video, Download) take a URL; choice and multi-star items take a list of options. Reorder items with the up/down arrows and collapse them while you edit. Above the item list, a definition sets its name, logo, header colour, one customer-facing and one or more internal print templates, and — with the Loyalty module on — Rewards earned for completing the survey (points granted on confirmation). You can also wire a definition to a Google Sheet for export.
The forms list
The Surveys list (/form under Master Data, or /crm/forms under CRM) shows every survey sent out.

Who can access this: Requires the Forms permission and module.
The toolbar has a form-definition filter (show only surveys built from certain definitions), a date range, and Copy / CSV buttons for the selected rows' links and answers. Per row you get a QR code, Show (open the public survey), Copy link, and Print (an internal template). Click a row to review the submitted answers in a read-only summary, adjust the status (draft/confirmed) or due date, and see the confirmed date.
Sending a survey
- Click + Create.
- Pick a Form Definition, one or more customers, and (optionally) a due date.
- Save — Gurita creates one survey per customer.
From the list, copying or CSV-exporting the links is handy for a mail merge, and the QR code is useful printed on a card or shown at a kiosk.
The public survey and QR lookup
Who can access this: Anyone holding the survey's personal link, or anyone who scans/opens the QR page for a trip — both work without a Gurita login.
The guest opens their link (no login) and answers each question — short/long text, date/time pickers, star ratings, single/multi-choice, image and video blocks — then clicks Save (keep as draft) or Confirm (lock the answers in).

Tip: For a QR code that isn't tied to one specific guest — for example printed on a table tent or posted on the boat — point it at the QR entry page for that trip instead. The guest types their email, and Gurita looks up (or offers) the matching survey.
